This page contains important information that will help your student have a successful year in physical education. Please check back frequently for updates and other information!
MEDICAL/ILLNESS NOTE
Please make sure to print out the Parent Note Template Form at the bottom of this section if your child needs to be excused from class.
If your student is sick or injured, you will need to complete a physical education parent note form (this form is posted on my website) so that I can modify what your student will do in class that day. Parent notes that excuse students from taking part in class work will be considered limited taking part classwork excuses. This limited taking part in classwork excuse will excuse the student from any activity that will affect their illness or injury. Only a doctor’s note can excuse a student totally from physical activities. Students are expected to be in their uniform when they have a parent or doctor’s note. In addition, students are responsible for any and all make up work. Most make up work can be found on my website under the “Make Up Work” section.
Locker Room Etiquette
Each student will be assigned a locker in his or her respective locker room. In addition, you have purchased a lock and a uniform for their daily use. Students are expected to use only the locker, lock, and uniform that belongs to them. Sharing is not allowed in the locker room. Please make sure that your student has his/her name in permanent marker on his/her uniform. If your student ever forgets his/her uniform, they will need to borrow uniform clothes from the locker room office.
Please be advised, Sequoia is not responsible for lost or stolen items. Students are responsible for the security of their belongings and should not bring ANY valuables to the locker room. The physical education teachers spend a great deal of time at the beginning of the school year teaching the students HOW to use the locker room safely and correctly. Students who fail to follow locker room policies can lose their locker room privileges.
Binders
Students will need to bring a working binder with paper, pen/pencil, and assignment notebook to class each class period. Failure to have the proper supplies will result in a drop in work habits mark and/or a lunch detention.
Grading
Your student will be graded on their grade level curriculum (including but not limited to: skills tests, rubrics, projects, written work, routines, etc.) and fitness (completion of, and improvement). Each month your student will be responsible for a class Fitness Log that will also require your child to be physically active outside of school time. In order for your student to earn FULL credit for the log, you will need to initial and/or sign it each month to verify all work done outside of class.
Citizenship/Work Habits Marks
Citizenship/Work Habits marks will follow the school-wide rubric found in the assignment notebook. Failure to adhere to the school/class rules may result in a drop in your student’s marks.
Physical Education Uniform (Dressing Requirements):
Students dress in a physical education uniform every day their class meets. Wearing your uniform allows you to move freely and safely. The Sequoia Middle School Physical Education uniform consists of:
1. Sequoia Physical Education Shirt (or a PLAIN light gray shirt - no logos)
Name must be clearly printed in the box on front of shirt.
Shirt must be tucked in at all times.
Any writing other than first and last name on uniform is not allowed and uniform will need to be replaced.
2. Sequoia Physical Education Shorts (or PLAIN navy blue shorts - no logos or stripes)
Name must be clearly printed in the box on the front left side.
3. Athletic Socks
4. Shoes with TIED Laces (for safety and performance)
We require that shoes worn to class have laces that are tied tightly and are appropriate for physical activity.
**Sweatshirts/Sweatpants may be worn during cold weather only. They must be worn OVER the physical education uniform. They are not to replace the uniform!
What happens if you forget your uniform?
You will dress in a set of clean loaner clothes provided in the locker room.
You will still be required to take part in class.
You may borrow one time per trimester without consequence. Borrowing multiple times in a trimester will result in detentions that follow the school-wide detention protocol.
What happens if you lose your uniform or lock?
Uniforms and locks are available in the main office. A lock must be used in the locker room at ALL times.
