**We must have new orders each school year for each medication.  Orders need to be dated June 1st or later each year.**

For your convenience here are a few of the most frequently used:

• Request to Administer

  • 6th Grade Students under 12 years old must have the Physician Health Care Provider Section signed for Adult Over The Counter medications (IE- Tylenol Extra Strength, Ibuprofen Tablets), not the Non Prescription Section. They will need to indicate the dosage and the frequencies, as well.
