**We must have new orders each school year for each medication.  Orders need to be dated June 1st or later each year.**

For your convenience here are a few of the most frequently used:

• Request to Administer

  • All Medications (including over the counter) left in the clinic require a physician signature. Please make arrangements to bring your student medications while waiting for orders, as needed. 

  • This is also the form for short-term medications. The clinic either must have it's own supply in a prescription package from the pharmacy, or parents will need to drop off and pick up the medication everyday if they don't want to come in to give on their own (ie: lunchtime dose).

• Self Carry Medication

  • Please complete this form if your student is in athletics, band, choir, dance, theater, or any group that might have Field Trips or Off-Campus events.
