Course and Program Forms
In order for course or program proposals to be added to a Curriculum Committee meeting agenda, proposals must have been approved by the responsible Dean and moved the next step in the workflow by the due date. Proposals should be submitted to the Dean before the due date, to allow time for review.
Starting Fall 2023, ALL new and revised credit courses and programs will be submitted using the Watermark Curriculum Management System (CMS). Non-credit proposals: NC courses and certificates will continue to use the current forms and submission process for the time being.

New programs cannot be marketed until they receive approval from Lane's accrediting agency, NWCCU
Financial aid cannot be approved for programs of 45 credits or more until they receive approval from the accrediting agency, Northwest Commission on Colleges and Universities (NWCCU)

Revise Course

Use this form for:
All course revisions (subject code, number, credits/hours, requisites, outcomes, descriptions)
Common Course Numbering (CCN) revisions
Attribute applications (i.e., Arts & Letters, Human Relations, etc.).
Attribute applications must be filled out separately and attached to the proposal.

New Course

Use this form for:
New courses, including any that are part of a program proposal (submit all courses separately)
Making 199/299 experimental courses permanent
Requests to reactivate courses

Inactive Course
Guide: How to Fill out the Inactive Course Form
Use this form for:
Requesting inactivation and to have the course removed from the catalog.
Departments are notified each year about courses that have not run for three years, but if you know you are discontinuing a course, it is best practice to have it removed from the catalog.

Use this form for:
Proposing annual program revisions.
CTE program check-ins will still be scheduled Oct-Feb to go over proposed changes and to discuss other program issues. Work with the Curriculum Team before using this form.

Use this form for:
Entering new program proposals
Note: BAS program proposals require additional documentation; see link below for details
Before you use this form, contact the Curriculum Team for training and to map out the timeline and estimated program launch date.
For new programs, you must include local and regional industry data. The state requires, at minimum, inclusion of one Occupation Profile for Lane County from the Oregon Department of Employment. If the Occupation Profile is missing any data, or you cannot find jobs related to your program, you will need to work with local industry partners to fill out and submit the Supplemental Occupation Profile form (one or the other is required).
Suspend Program
