Student Wellness Request: If you are a student or a friend of a student who would like to receive a wellness check-in, please click on the link below or scan the QR code to fill out a form. Please fill out this form out if you would like to meet with Wellness Staff. Requests will be responded to within 48 hours or the next regular school day, if submitted during a weekend or holiday.

Student concerns related to academics, attendance, and/or behavior, please complete a KTJUSD Student Support Request Form.
For physical health, mental health, substance use, and/or community resource related student concerns, please complete the KTJUSD Wellness Request Form.
