School Quality Survey Administration
Announcement Update as of Sept 5, 2025
The SY 2025-26 SQS will be administered from January 7 to March 12, 2026.
Both Parent and School Personnel Surveys are currently closed and will not accept any responses until January 6, 2026.

Survey Format Options: Online vs. Hard Copy
Benefits of Online Surveys
Easy to distribute via email, website, or QR code.
Individual responses are anonymous, but the overall school response rate can be monitored.
Benefits of Hard Copies
Parents can review the questions and complete the survey as soon as they receive a paper copy.
Enables monitoring of response counts by grade or class.
Challenges with Hard Copies
Many parents did not return surveys on time.
Some surveys were incomplete or marked incorrectly, making results unusable.
Surveys printed in the wrong size or missing school codes/names could not be included in the results.
Hard Copy Requests Ordering Timeline
Place your order by Friday, September 5, 2025 , using this spreadsheet. Type “0” to confirm your school do not need any copies.
Hard copies will be printed and delivered by December 20, 2025.
Due to limited staff and resources, we cannot process additional requests of more than 100 copies after January 6, 2026.
