Please fill out the SY 2025-2026 SQS distribution count spreadsheet by Feb 10, 2026.
The SY 2025-26 SQS will be administered from January 7 to March 12, 2026.

This survey is for K-12 parents only. Pre-K parents do not need to fill it out.
Survey Administration Format
Students: The SQS questions for students are embedded in the Student Perception Survey, so students do not need to take any additional action during this period.
Parents: can choose to complete the survey online or answer a hard copy.
All school personnel: complete the survey online.
If your school collects hard copies from parents, please ensure that all hard copies arrive at the Accountability Section Office by March 17, 2026. Our vendor will collect the final batch on March 18, 2026.
Request Extra Hard Copy Between December 22, 2025 and February 27, 2026.
Use this form to request extra hard copies if it is need.
Due to limited staff and resources, please request no more than 150 copies. Please note, we have run out of Business Reply Envelopes, so schools will need to collect the completed surveys and send them back to the Accountability Section office. Thank you!

Hawaiian, Ilokano, Japanese, Korean, Marshallese, Samoan, Spanish, Tagalog, Thai, Tongan, and Vietnamese.
Note: Parents' surveys must be printed in legal size paper for scanning. Please use the right size of paper.
