Parent/Guardian Survey Distribution Guidelines
Schools should distribute parent letters and/or survey packets to all parents/guardians of students in Grades K-12.
One parent/guardian may complete one survey for each K-12 student.
Do not distribute surveys based on "oldest" or "youngest" students; ensure each student receives a parent letter or survey packet.
If parents are separated/divorced, the primary caretaker should complete the survey for the student. (Only one survey packet
should be given per student.)
The parent letter and survey are available in 15 languages. These translated surveys will be accessible in December at http://arch.k12.hi.us/reports/sqs. (Use the drop-down menu to select the current school year and the desired language for
the parent letter or survey.)
Communicating with Parents
Decide how to inform parents about completing and returning the School Quality Survey (SQS).
Consider innovative methods to maximize survey return rates.
Online Survey Responses
Most parents will complete the survey online.
Hard Copy Survey Responses
Do not write or print personally identifiable information on the surveys or envelopes to protect anonymity.
Do not open returned parent surveys; they must be returned in sealed envelopes.
If surveys are returned with or without an envelope, place all surveys in a single large envelope and return them via
the DOE courier to:

If a student loses a survey packet, provide them with another or ask the parent to call (808) 307-3650 to request a replacement.

Any incentives to increase survey return rates must not be tied to students' grades or academic extra credit.
Tracking Parent Surveys
Keep track of the exact number of parent surveys distributed.
The number of surveys distributed should closely match your January enrollment count for students in Grades K-12.
