This letter and survey are also available in the following 15 languages: Cebuano, Chinese (Simplified and Traditional), Chuukese, Hawaiian, Ilokano, Japanese, Korean, Marshallese, Samoan, Spanish, Tagalog, Thai, Tongan, and Vietnamese. 

Note: Parents' surveys must be  printed in  legal  size  paper for scanning. Please use the right size of paper. 

Survey Format Options: Online vs. Hard Copy

Benefits of Online Surveys

• Easy to distribute via email, website, or QR code.

• Individual responses are anonymous, but the overall school response rate can be monitored.

Benefits of Hard Copies

• Parents can review the questions and complete the survey as soon as they receive a paper copy.

• Enables monitoring of response counts by grade or class.

Challenges with Hard Copies 

• Many parents did not return surveys on time.

• Some surveys were incomplete or marked incorrectly, making results unusable.

Surveys printed in the wrong size or missing school codes/names could not be included in the results.
