Transcript Requests: Please read the transcript information and instructions from our Registrar's Office BEFORE downloading this form fillable request form. (If you have trouble accessing document please sign in using your school account.) All seniors should turn in their completed transcript request forms to C&CC.
If you are under the age of 18 and would like to apply for a job you will need a work permit. They can be found online at the link to the left. You will need to provide an email address and the last four digits of your social security number. Additional documents you may need (as applicable): Hawaii driver's license, State of Hawaii ID, birth certificate, immigration record (aliencard, visa), passport, or military ID. Please note: there is one form for 16-17 yr olds and a different one for 14-15 yr olds.
