Phase II is due Nov. 1 and includes two elements. 

1) School Assurances

Resources for completing the School Assurances: School Assurances Template, School Assurance Items & Suggested Response Guidance (located on the Phase II page), and the KDE Schools User Guide

2) Needs Assessment

Resources for completing the Needs Assessment: Needs Assessment Template, Prior CSIP, JCSP Needs Assessment Example, CSIP Activity Database, and the KDE Schools User Guide

Submitting Phase II

1. Confirm that School Assurances and the Needs Assessment both show Marked as Completed.

2. A school administrator should select Phase Two Complete at the bottom of the plan.

IMPORTANT: The image below illustrates the button you use to select that will indicate your Phase II is Completed. Until this button is selected, your Phase II will be listed as In Progress.
