How to apply for Working Papers: 

Review Department of Labor Guidelines for working papers.

Print a copy of the"AT-17: Application for Employment Certificate".

Read the "General Information" page carefully and fill out the form:

• Part I: Completed by parents/guardians.

• Part II: Prepare documents verifying age (birth certificate, driver's license, etc.)

• Part III: Obtain a copy of most recent health physical (must be within the last 12 months and state that there are no limitations to work/activity).  This can be completed by your school's nursing department.

• Part IV: To be completed by prospective employer.

• Part V: Must be signed by a school official in the main office.

• Part VI: To be completed after submission by issuing official.
