Students are required by New York State to be up-to-date with immunizations to attend school. Minimum requirements for vaccines are listed below. Please send a copy of your child's most recent vaccine record before they start school. Please note, in 2019, NYS removed all religious exemptions for school-required vaccines.
Students who are not up-to-date with required immunizations may be subject to exclusion from school.
At this time, Indian River does not offer vaccine administration on any campus. Immunizations can be given by your child's healthcare provider, at Guthrie Clinic (military only), or at Jefferson County Public Health.
Immunization Requirments

Requirements prior to entering Pre-Kindergarten (Head Start, UPK, etc.):
Diptheria, Tetanus and Pertussis (DTap): 4 doses
Polio (IPV/OPV): 3 doses
Measles, Mumps and Rubella (MMR): 1 dose
Hepatitis B: 3 doses
Varicella: 1 dose
Haemophilus influenzae (Hib): At least 1 dose
Pneumococcal (PCV): At least 1 dose
Requirements for entering Kindergarten through 5th grade:
Diptheria, Tetanus and Pertussis (DTap): 1 dose
Polio (IPV/OPV): 1 dose
Measles, Mumps and Rubella (MMR): 1 dose
Varicella: 1 dose
Requirements for students at age ten (10):
Tetanus, Diptheria and Pertussis (Tdap): 1 dose
Requirements for entering grades 7-11:
Meningococcal (MenACWY): 1 dose
