Whether your student is coming from another school or starting out their education journey in Pre-K, we have all the information and health links you need to ensure your family is ready to join our WOLF Pack!

Step 1: Immunizations

Students are required by New York State to be up-to-date with immunizations to attend school. Please send a copy of your child's most recent vaccine record before they start school. Please note, in 2019, NYS removed all religious exemptions for school-required vaccines.

Students who are not up-to-date with required immunizations may be subject to exclusion from school.

At this time, Indian River does not offer vaccine administration on any campus. Immunizations can be given by your child's healthcare provider, at Guthrie Clinic (military only), or at Jefferson County Public Health.

Step 2: Physicals

For new students, we will need a copy of a most recent physical. For a physical to be valid, it must be dated within 12 months of the student's first day of school and signed by a healthcare provider licensed in New York State (or a military healthcare provider if coming from out of state). Physicals are also required for students to play sports.

For students who need medication during school hours, a medication order is required with a signature from both the healthcare provider and parent/guardian. This includes prescription medication, over-the-counter pills, antibiotics, inhalers, creams/ointments, ear/eye drops and nasal sprays.

Medications must be delivered to the school and picked up by the parent/guardian. No student should be carrying medications on school grounds unless they have a self-carry order from their provider to do so.

For children under 6, a copy of their most recent lead screening should be attached to their physical.

Step 3: Care Plans

For students with chronic health conditions, our nursing staff will ask for a care plan to be completed and attached to their physical. This care plan provides specific information about your child's unique needs to ensure we are providing optimal care. Please choose the care plan you need from the list provided and take it to your child's healthcare provider to be completed.

Step 4: Dental Certificates
