Health Office Forms . . . 

• Annual Student Health Questionnaire - kindly fill out at the start of each academic year so we can best care for your child at school. Form is part of online registration.

• Immunizations -  If your child has had vaccinations in the past year, kindly provide us with a copy.  If you choose NOT to vaccinate your child, we will need a Religious Exemption form signed at the start of each school year. 

• Allergies/Asthma – If your child has an allergy or asthma, please be in touch with the health office so we can have a plan in place here at school.
