PRE-APPLICATION FOR HOUSING
Complete the Hillsboro Housing Authority application for public housing in Hillsboro, Kansas
All apartments are one-bedroom units and may be occupied by no more than two people.
DECLARATION OF SECTION 214 STATUS
Complete the required citizenship/eligible immigration status declaration.
HUD 9886-A FORM Authorization for the Release of Information/Privacy Act Notice

HUD-92006 FORM — Supplement to Application for Federally Assisted Housing Form
This form gives you the opportunity to identify a person or organization who may be contacted to assist in providing services or resolving issues that may arise during your tenancy or application process. You may choose the option to not provide additional contact information.
PHA RELEASE OF INFORMATION FORM
This form gives the Hillsboro Housing Authority permission to contact third parties, such as employers, financial institutions, benefit providers, landlords, or other agencies, to verify information needed to process your housing application.
EIV AND YOU FORM
This HUD form/information guide explains the Enterprise Income Verification (EIV) system, how income and benefit information may be used to verify your household’s eligibility and rent, and your rights and responsibilities regarding the information contained in EIV.
COMMUNICATION PREFERENCE FORM
This form lets you tell the Hillsboro Housing Authority how you prefer to receive important communications, such as by mail, email, phone, or text message. Providing your communication preferences helps us contact you about your application, waiting list status, appointments, and other housing-related matters.
SUBMIT REQUIRED DOCUMENTS
DOCUMENT UPLOAD
Please submit the required documents for each household member included on your application. These documents are used to verify identity, household composition, and eligibility for housing assistance.
Please provide:
Govt. Issued Photo Identification for each adult household member
Social Security Number documentation for each household member
Birth Certificate for each household member
