Work Permit Request Form

The below information will be verified prior to issuing an intent to work form.

1. GPA will need to be verified. (Must have a 2.0 and No "F" grade on last report card)

2. Permit will be emailed to student upon form completion.

3. Have your new employer complete permit form. 

4. Submit completed permit form to Mrs. Aguilar by email.
