  *Please note the Curriculum Office must receive all forms a minimum of 14 days prior to the field trip. No field trip will be approved following 5/1*

  • Please complete this entire form. It will automatically be submitted to the Nurse Lead, your specified building Principal, and then the Curriculum Office.

  • If the field trip involves an overnight stay, is beyond a 120-mile radius of Haverhill, or requires unconventional travel such as by air or on water, the approval of the Haverhill School Committee will be required. Please allow up to 60 days for processing.

  • Permission Slips may not go out to students until the request has been approved.

  • Notify your cafeteria staff once the date has been approved.

After School Extracurricular Nursing Form

To ensure proper nursing coverage and coordination, we are implementing a process for requesting nursing for extracurricular activities. This process is similar to the field trip request process and is necessary to prevent last-minute scheduling issues and ensure nurse approval.

All requests must be submitted using the designated form at least seven (7) days in advance. Requests not submitted through this process will not have nursing coverage.
