Immunization and Health Exam Requirements
IMMUNIZATIONS/HEALTH SCREENINGS FOR THE 2024-2025 SCHOOL YEAR
You can start scheduling immunizations and health screenings for your child for the 2024-2025 school year. Students must have the following exams and/or immunizations before August 15th, 2024. Students will not be allowed to attend school until these screenings are complete. Appointment cards will not be accepted as a pass into school.
Physical exams: A student must have a physical examination (school) within one year before entering a preschool program, kindergarten, sixth or ninth grades.
Immunizations: Students are required to show proof of basic immunizations and required boosters for
Diphtheria, Tetanus, Pertussis, Polio, MMR (Measles, Mumps and Rubella), Varicella, and the following:
PreK: Haemophilus Influenzae Type B
PreK and 6th through 12th grades: Hepatitis B
Children 2 years up to 5th birthday (24-59 months): Pneumococcal Vaccine (PCV)
PreK and Kindergarten: must show proof of lead screening
Kindergarten through 12th grade: must show proof of two vaccinations for Varicella (chickenpox)
12th grade: Must show proof of receiving two doses of meningococcal vaccine (meningitis) with the second dose received on or after the 16th birthday. If the first dose was given after the age of 16, then only one dose is required.
Vision exams:
Kindergarten: Students entering kindergarten must have an eye exam.
Dental exams:
Kindergarten, 2nd, 6th, and 9th grades.
Objections: A child’s parent or legal guardian who objects to health or dental examinations or immunizations must submit an Illinois Certificate of Religious Exemptions form. For the correct form required by the state, please contact your health care provider.

Immunizations may be emailed to nurses@harlem122.org or mailed to/dropped off at the appropriate school.
Illinois Certificate of Religious Exemption must be completed by the guardian and the child's health care provider.
