CHANGE OF ADDRESS
Please complete the Proof of Residency Affidavit (below) and provide a minimum of two (2) verification documents (same as residency verification). Residency verification documents can be emailed to registration@harlem122.org or dropped off at the Harlem Administration Center (8605 N. 2nd St., Machesney Park, IL).
What If I Am Unable To Provide a Required Residency Document?

Required if a parent or guardian seeking to enroll a child needs evidence of residency when a signed lease is unavailable. This includes an expired lease that has automatically become a month to month tenancy. Other documents will also be required to establish residency.
Top portion to be completed by parent or legal guardian and bottom portion to be completed by Landlord
Return this Affidavit along with proof of prior month payment and a document from Category B

Required if a parent or guardian seeking to enroll a child is living with a District Resident, such as friends or relatives, and they cannot provide sufficient proof of residency in their own name.
To be completed by the parent or legal guardian and the District Resident
District Resident is required to provide a minimum of two (2) verification documents required to establish residency
Parent or legal guardian will be required to provide a minimum of one (1) verification document from Category B (see Residency Documents for Address Changes below)
The School District reserves the right to evaluate the evidence presented, and merely presenting the items listed in this procedure does not guarantee admission

Verification documents from Category A and Category B must state the name of the District Resident
The service address and mailing address must match
No hand-written letters will be accepted as Legal Rental Agreements or Affidavits (see board approved forms)
Board approved forms must be used
Acceptable Verification Documents
A minimum of two (2) documents are required to establish residency, one (1) from each category. If a utility bill is used to satisfy Category A, the same bill cannot be used to satisfy Category B. No hand written letters will be accepted as Legal rental Agreements or Affidavits. Board approved forms must be used. Documents must be presented at the time of registration. A Post Office box WILL NOT be accepted.

CHANGE TO TRANSPORTATION
Please contact our Parent Resource Secretary, Elizabeth Hare (815-654-4500 ext 1040), to request the transportation change. Please note that it can take up to 5 business days for the Transportation Department to contact you with a new bus route.
