Athletic Participation (Appendix 'A')
In order to participate in any organized sports, a completed Appendix 'A' form must be submitted to the coach.
Transportation Form (Parent Drivers)
In order for parents to be volunteer drivers (taking students to away games) this form must be submitted to the coach.

In order to be a volunteer for any organized sport, a Police Check must be on file with the office.
