ELO Contract & Liability Forms
These forms will need to be filled out before a student can complete their ELO.
Each student will be required to create a Design Plan for their ELO with the help from a Coach. This plan outlines your goals and essential questions for the ELO. If you would like to see an example of a filled out Design Plan, you can find one in the ELO Syllabus linked above. This Design Plan requires signatures.

This is where students record their hours for their ELO placements to track progress and gain credit.
This is the final written reflection prompt. It will be due by the last meeting with the ELO Coach. During the last meeting, there will also be an evaluation and a final determination of credits earned.
Complete this form as your "application" for the Independent ELO program. Once you have completed the form, an ELO Coach will be in contact with you shortly to set up your first meeting.
