RE01 - General Request Form (คำร้องทั่วไป)

• Request to cancel the enrollment. This request has to pay a fine 100 THB/Time according to the Regulations of Burapha University(3). (ขอยกเลิกการลงทะเบียนเรียน ซึ่งมีค่าปรับที่นิสิตต้องชำระ 100 บาท/ครั้ง ตามข้อบังคับมหาวิทยาลัยบูรพา(3))

• Request for pay in advance for graduation. (ขอชำระค่าธรรมเนียมการศึกษาล่วงหน้าเพื่อสำเร็จการศึกษา)

RE03 - Request form for transcript/certification letter/Student ID Card (คำร้องขอใบแสดงผลการเรียน/ใบรับรอง/บัตรนิสิต)

• Student ID Card (บัตรนิสิต) --Fee 70, 100 THB/Card

• Transcript (ใบแสดงผลการศึกษา) --Fee 100 THB/Copy

• Certification of Student Status (ใบรับรองการเป็นนิสิต) --Fee 100-200 THB/Copy

RE04 - Request Form for Student Name, Title or Address Change (คำร้องขอเปลี่ยนชื่อ นามสกุล คำนำหน้า หรือที่อยู่)

• Name or Title Change (เปลี่ยนชื่อ หรือคำนำหน้า)

• Address Change (เปลี่ยนที่อยู่)

RE05 - Late Registration Form (คำร้องขอลงทะเบียนล่าช้า)

• This form can apply after the last day of Online Registration but not more than 1 week from the day that semester start (The semester that you request Late Registration) or by the day that is announced in Academic Calendar.

• This form requires the advisor approval sign before, then instructors approval sign for each courses.

• This request shall a fine according to the Regulations of Burapha University(3).

1. Late from the last day of Registration, not more than 1 week, 500 THB/Time.

2. Late from the last day of Registration for more than 1 week, 100 THB/Day.

• สามารถยื่นขอลงทะเบียนเรียนล่าช้าได้หลังจากพ้นช่วงการลงทะเบียนรายวิชาออนไลน์ แต่ไม่เกิน 1 สัปดาห์หลังเปิดภาคการศึกษาที่นิสิตขอลงทะเบียนเรียนล่าช้า หรือตามกำหนดวันที่ประกาศในปฏิทินการศึกษา

• การขอลงทะเบียนล่าช้า จะต้องได้รับการอนุมัติให้ลงทะเบียนจากอาจารย์ที่ปรึกษาก่อน จากนั้นจึงจะสามารถขออนุมัติจากอาจารย์ผู้สอนแต่ละรายวิชา

• การขอลงทะเบียนล่าช้า จะมีค่าปรับที่นิสิตต้องชำระ ตามข้อบังคับมหาวิทยาลัยบูรพา(3)

1. ขอลงทะเบียนล่าช้านับจากวันสุดท้ายของการลงทะเบียน แต่ไม่เกิน 1 สัปดาห์ มีค่าปรับ 500 บาท/ครั้ง

2. ขอลงทะเบียนล่าช้านับจากวันสุดท้ายของการลงทะเบียนเกิน 1 สัปดาห์ มีค่าปรับ 100 บาท/วัน

RE05-1 - Late Add-Drop Registration Form (คำร้องขอเพิ่มลดรายวิชาล่าช้า)

• This form can apply after the last day of Add-Drop Online or by the day that is announced in Academic Calendar but not more than 1 week.

• This form requires the Instructors Approval and Advisor Sign.

• This request shall pay a fine according to the Regulations of Burapha University(3).

1. Late from the last day of Add-Drop online, not more than 1 week, 500 THB/Time.

• สามารถยื่นขอเพิ่มลดรายวิชาล่าช้าได้หลังจากพ้นช่วงการเพิ่มลดรายวิชาออนไลน์ โดยมีระยะเวลาดำเนินการไม่เกิน 1 สัปดาห์ หรือตามกำหนดวันที่ประกาศในปฏิทินการศึกษา

• การขอเพิ่มลดรายวิชาล่าช้า จะต้องได้รับการอนุมัติให้ลงทะเบียนจากอาจารย์ผู้สอน และอาจารย์ที่ปรึกษาลงนาม

• การขอเพิ่มลดรายวิชาล่าช้า จะมีค่าปรับที่นิสิตต้องชำระ ตามข้อบังคับมหาวิทยาลัยบูรพา(3)

1. ขอเพิ่มลดรายวิชาล่าช้านับจากวันสุดท้ายที่มหาวิทยาลัยประกาศ กำหนดเพิ่มลดรายวิชาออนไลน์ ไม่เกิน 1 สัปดาห์  มีค่าปรับ 500 บาท/ครั้ง หลังจาก 1 สัปดาห์ คิดค่าปรับเพิ่มวัยละ 100 บาท

RE06 - Request form for Change Faculty or Major (คำร้องขอเปลี่ยนสาขาวิชา หรือย้ายคณะ)

• Students can request to Change Faculty or Major only 1 time.

• Students shall contact the new Faculty or new Major before completing this form. 

• Only for Student ID 64 and above. This request shall pay a fee according to the Rules of Burapha University(4).

1. Change Faculty 1,000 THB/Time (Foreigner Student, 2,000 THB/Time)

2. Change Major 2,000 THB/Time (Foreigner Student, 4,000 THB/Time)

• นิสิตสามารถยื่นคำร้องขอเปลี่ยนสาขาวิชา หรือย้ายคณะได้ 1 ครั้งเท่านั้น

• นิสิตจะต้องติดต่อกับคณะ หรือสาขาวิชาที่นิสิตจะขอย้ายก่อนกรอกแบบฟอร์มนี้

• เฉพาะนิสิตรหัส 64 เป็นต้นไป จะมีค่าธรรมเนียมในการดำเนินการ ตามที่กำหนดในระเบียบมหาวิทยาลัยบูรพา(4)

1. การขอเปลี่ยนสาขาวิชา 1,000 บาท/ครั้ง (นิสิตต่างชาติ 2,000 บาท/ครั้ง)

2. การขอย้ายคณะ 2,000 บาท/ครั้ง (นิสิตต่างชาติ 4,000 บาท/ครั้ง)

RE08 - Request Form for Being Reinstated to Student Status (คำร้องขอคืนสภาพการเป็นนิสิต)

• If student had retired by not completing enrollment (Not pay a Tuition Fee by the period that announce in Academic Calendar) and want to Request for Being Reinstated to Student Status. Students shall pay a tuition fee and fine according to the Regulations of Burapha University(1).

• Only for Student ID 64 and above. This request shall pay a fee according to the Rules of Burapha University(4). Request for Being Reinstated to Student Status 2,000 THB/Time.

• หากนิสิตพ้นสภาพเนื่องจากลงทะเบียนไม่สมบูรณ์ (ไม่ชำระค่าธรรมเนียมการศึกษาตามช่วงวันที่กำหนดในปฏิทินการศึกษา) และต้องการขอคืนสภาพการเป็นนิสิต นิสิตจะต้องชำระค่าธรรมเนียมการศึกษาที่คงค้างอยู่ รวมทั้งค่าปรับตามที่กำหนดในระเบียบมหาวิทยาลัยบูรพา(1)

• เฉพาะนิสิตรหัส 64 เป็นต้นไป จะมีค่าธรรมเนียมในการดำเนินการ ตามที่กำหนดในระเบียบมหาวิทยาลัยบูรพา(4) ในการขอคืนสภาพการเป็นนิสิต ครั้งละ 2,000 บาท

RE10 - Request Form for Leave of Absence (คำร้องขอลาพักการเรียน/รักษาสภาพ)

• Students can apply this form for Leave of Absence for only 1 Semester/Time.

• Student First-Year can't apply this form for the First Semester.

• Students who already apply this form shall pay a Tuition Fee by the period announced in Academic Calendar except Student ID 64 and above. Because a Tuition Fee was a flat rate as according to the Announcement of Burapha University(2) and the Rules of Burapha University(4).

• Only for Student ID 64 and above. This request shall pay a fee according to the Rules of Burapha University(4). Request for Leave of Absence 2,000 THB/Semester (Foreigner Student, 4,000 THB/Semester)

• นิสิตสามารถยื่นคำร้องขอลาพักการเรียน/รักษาสภาพได้ครั้งละ 1 ภาคการศึกษาเท่านั้น

• นิสิตชั้นปีที่ 1 ไม่สามารถยื่นคำร้องขอลาพักการเรียน/รักษาสภาพในภาคการศึกษาแรกได้

• นิสิตที่ยื่นคำร้องขอลาพักการเรียน/รักษาสภาพ ต้องชำระค่าธรรมเนียมการศึกษาตามช่วงที่กำหนดในปฏิทินการศึกษา ยกเว้นนิสิตรหัส 64 เป็นต้นไป เนื่องจากเป็นการชำระค่าธรรมเนียมการศึกษาแบบเหมาจ่าย ตามที่กำหนดในประกาศมหาวิทยาลัยบูรพา(2) และระเบียบมหาวิทยาลัยบูรพา(4)

• เฉพาะนิสิตรหัส 64 เป็นต้นไป จะมีค่าธรรมเนียมการขอลาพักการเรียน/รักษาสภาพ ภาคการศึกษาละ 2,000 บาท (นิสิตต่างชาติ ภาคการศึกษาละ 4,000 บาท) ตามที่กำหนดในระเบียบมหาวิทยาลัยบูรพา(4) 

RE11 - Request Form for Resignation (คำร้องขอลาออกจากการเป็นนิสิต)

RE14 - Request Form for Course Transfer (คำร้องเทียบโอนผลการเรียน)

RE24 - Request to extend the period of payment (คำร้องขอขยายเวลาชำระเงิน)
