Information on Health Petition requirements including Third Party vendors
What is CastleBranch and how do I purchase?
A secure third party website to manage three areas: Criminal Background checks, Drug Testing & Medical Document Manager (Health Records) CastleBranch is needed to meet clinical agency requirements & enroll in your technical course classes.
If a student is interested in entering a Health Program they must complete a BACKGROUND CHECK ACKNOWLEDGEMENT FORM in order to register for classes. This is completed as part of the Admission process and prior to petitioning
After you have * petitioned and been accepted to start the program, MATC's HEALTHCARE PATHWAY will email you with the specific package code or codes to purchase your program's CastleBranch requirements: Purchases are made under "Place Order" in your CastleBranch account.
One exception: There is no petition requirement for the Nursing Assistant program. CastleBranch requirements will be required to complete
Students pay directly to CastleBranch for the service.
What is required in CastleBranch & how often do I need to complete each requirement?
Students who have petitioned and been accepted into a Health Program will be notified by the School of Health Sciences to complete their remaining CastleBranch requirements. After you've purchased your requirements, CastleBranch will notify you via email when requirements are about to expire. Always include your lab documentation records as an upload, along with the MATC forms.
You may find your lab records on the Wisconsin Immunization Registry.
Health Requirements (Medical Document Manager) = varies by program
HIPAA training - Lifetime (Disregard the Recertification Notice from the Vendor)
TB - yearly
Influenza - yearly
Drug Test - yearly (Download Drug Test form from CastleBranch and take to Quest Diagnostic Labs as indicated on form )
CPR - two years (American Heart Association BLS Provider) See list of vendors below.
Physical - 3 years (The Physician needs to sign the MATC form)
Tentanus - 10 years
Hep B - Lifetime
MMR - Lifetime
CNA (Certified Nursing Assistant for Nursing Students Only) If Licensed, access your Wisconsin CNA license via https://wi.tmuniverse.com/ and "Search Wisconsin Registry" by your Social Security Number. Otherwise you need to upload course completion documentation.
How do I find the CastleBranch Forms?
The majority of documents are specific to each HEALTHCARE PATHWAY program. Go to the HEALTHCARE PATHWAY webpage & select your program , click “CastleBranch Requirements & Forms”
There are some exceptions and clarifications:
The Criminal Background check requires the completion of the (BID) Background Information Disclosure form. The form is located and available to download from your CastleBranch account AFTER you purchase the Criminal Background check. The form requires a hand signature, NOT electronic signature.
The (CBC) Criminal Background Check & Self Disclosure Verification Form. This form is located in your CastleBranch Medical Document Manager (Health Records) section . The form should be completed and uploaded AFTER you've received your Criminal Background results and uploaded your BID form. Results are found in your Castle Branch account.
The (VPO) Verification Petition Orientation form. This form is can be downloaded and completed from your Medical Document Manager (Health Records) section. It is one of the first deadline requirements after you purchase your Medical Document Manager.
Drug Test A downloadable registration form populates in CastleBranch after students purchase a drug test package. The student will bring the form with them to perform their drug test. The locations for drug testing will be on the registration form. Results are sent by the Drug Testing center to your CastleBranch account.
Drug Test Verification Form This form is found with the majority of your other health requirement forms under CastleBranch Requirement & Forms. It should be completed AFTER you receive your results from your Drug Test. Results are found in your Castle Branch account.

Where can I go to complete the CastleBranch Medical Requirements?
You may find your lab records already on the Wisconsin Immunization Registry
Your personal physician might also administer or have records of your CastleBranch requirements.

Criminal Background Check - pay every two years
Medical Document Manager - Once a lifetime. If you change to a new program, CastleBranch will "matriculate" (add) your new document manager/program code at no charge. You will need to reupload all your documents to the new program fields.

Computer Room H126 (next to the Petition Office) can offer assistance with uploading your documents to CastleBranch. Assistance available 8:00 - 11:30 and 2:00 - 4:00 pm. The Computer Room may be closed for exams or orientations. You can also scan at the HEC building or C278
How do I access HIPAA training and complete the CastleBranch requirement?
NEW HIPAA Training website is https://nalearning.org/hipaa/MATC provided by the North American Learning Institute
• Create an Account
• Training cost is $15
• One hour of minimal training for course. Must preview all pages, cannot skip to post test and will time out if page is left open and no activity recorded. You can stop and start course. Extra Authentication for log in.
• Must score at least 70% or greater for successful completion on Post Test. You can retake Post Test to pass.
• Upload Successful Course Completion Certificate to CastleBranch
• Course can be taken on Desktop, Laptop, Tablet or Phone
