CREATING A GOOGLE FORM AND QUESTION TYPES.

To apply specific settings to Google Forms, click on the gear icon in the upper right hand corner 

Here you can set your general settings by limiting to one response, collecting email addresses, etc, and also your presentation settings. The settings shown below are the default settings, but you can change them to meet your specific needs.
