Student ID, VOE, or Another Form?

HOW TO REQUEST a Student ID:

Need a student ID? Follow steps one and two, then please allow at least a week for your ID to arrive via USPS.

Step 1. Pay your $5 Student ID fee in RevTrack.

Step 2. Use this form to request your ID from the iUP Office. Your RevTrack must be paid before we can create the ID. Thank you!

Complete this form to request documents such as a VOE, work permit, letter of good standing, permission slip, progress report, etc.
