Immunizations

Minnesota law mandates that children enrolled in a school program be current with specific immunizations, or provide a legal exemption to the school. Failure to comply with immunization requirements or provide exemption documentation may result in exclusion from school. View the Immunization Requirements form below to see if your child is up to date. It is recommended that you call your child’s physician to schedule an appointment for immunizations. The Wadena County Health Department offers low cost immunization clinics. Dates and time of immunization clinics can be obtained by calling the Wadena County Health Department at (218) 631-7629. Office hours are Monday – Friday (8:00 a.m. – 4:30 p.m). More information may be obtained by calling the Minnesota Immunization Hotline at 1-800-657-3950. If you have any questions regarding your child's immunization status, please contact the school health office.
