Form 1095-A, B, or C: What You Need to Do with thESE Forms

These are forms regarding health insurance coverage that will be sent to you if they are required.  For IRS information about each of the forms, see Questions and Answers about Health Care Information Forms for Individuals (Forms 1095-A, 1095-B, and 1095-C)

Forms 1095-B and 1095-C are not needed for your tax return and do not need to be submitted. Students who have their health insurance through their school will not receive a 1095-A.

What are 1095-A, B & C?

Form 1095-A (Health Insurance Marketplace Statement) is sent to those who purchased health insurance through the Health Insurance Marketplaces. The form is sent to you by the Health Insurance Marketplace. See About Form 1095-A for more information. 

If you received this form and you are a nonresident alien, you will need to use IRS Form 1040NR (the long form). See the instructions for IRS Form 1040NR

Form 1095-B (Health Coverage) is sent to those who had health insurance that was not purchased through the Health Insurance Marketplaces. The form is sent to you by the health coverage provider (Insurance company, government agency, employers, etc). See About Form 1095-B for more information.

Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) is sent to those who had health insurance through at was not purchased through the Health Insurance Marketplaces.
