This page includes links to all necessary forms and documents that need to be completed before an ERG is formally recognized by Fleet Feet.
Step 1. Complete the Request form - This is the first step for anyone interested in forming an ERG. At least one ERG co-Leader should fill out the interest form. While formal documentation isn't required at this stage, ERG leaders should begin discussing their desire to serve as an ERG Leader before completing the interest form. After submitting the form, a member of the HR team will reach out to schedule a brief check-in to discuss the process. Any potential co-leaders and/or members can be invited to join as well.
Step 2. Manager Employee Participation Agreement—Each co-leader should review and complete this document in collaboration with their manager. Then, each co-leader's document will need to be uploaded to the application.
Step 3. The items below should be completed collaboratively with ERG Leaders and the Executive Sponsor. Members should also be allowed the opportunity to contribute during this process.
Charter Template - The charter should be completed as a group activity with founding members and co-leaders. Review charter guidelines and examples here. This document will be uploaded to the application.
Roster Template - This sheet will also be uploaded to the application. However, it will also serve as the group's official roster of members and should be updated accordingly.
Employee Resource Group Application - After completing the documents above, groups with at least 10 members (including co-leaders) should complete this application, including all requested documents.
