It is STATE LAW that you must have a Driver's School Compliance Verification Form (AKA No Pass/No Drive Form) featuring our school seal to verify that the student has FEWER THAN 9 unexcused absences for the previous semester AND has passed 6 out of 8 (or equivalent) classes in the previous completed semester in order to take the written driver's permit test. Currently we are required to check the second semester of the 2024-2025 school year. The student must have the ORIGINAL paper form which bears the embossed school seal in order to take their permit test. You do not need anything from the school if you already have your permit and are taking your road test. You also do not need the form if you have graduated OR if you are 18 years old or older.
If you are 14 years old and want to take your test on your 15th birthday, go ahead and request your form 1-2 weeks before your birthday. We will date it for your 15th birthday and it will be valid for 60 days.
Once you submit your request via the Google form (click the button below), your attendance and grades will be verified. You will be notified by email when your form is ready. Please allow up to two school days for your request to be processed. (NOTE: If school is not in session and the Guidance Office is closed, we will not be processing requests.) If your student is in 9th grade, processing time may be delayed.
Also, be advised that at the end of each semester we are required to report students who are under 18 years old with 9 or more unexcused absences and/or students who failed more than 2 classes to the state and the permit or driver's license will be revoked.
