We will need  documentation from you before you can begin your clinical practicum this upcoming Fall (Residential) or this upcoming Spring (Online).  ALL documents will need to be uploaded into Calipso. These are the items you need to be completing and securing. PLEASE start this now so this can be done quickly. Calipso access should have been granted to everyone by now. If you have not received an email to set this up,  please contact Mrs. Hatcher (ahatcher@faulkner.edu) if you are on-ground or Dr. Rine (krine@faulkner.edu) and Dr. Emily Linck (elinck@faulkner.edu) if you are online. This needs to be completed by August 2nd.

1. Current TB test (current is defined as one completed in June or July of this year, preferably)

2. Vaccination records

• MMR

• meningitis

• Hep B

• flu (recommended)

3. A copy of your health insurance card (if you have one)

4. If you have received the COVID-19 vaccine and any additional boosters, please send that information as well.*

*At this time, Faulkner does not require the vaccine, but we will want to know if you have taken the vaccine in order to determine potential placement sites that do require it.
