Dear Parent/Guardian of an Incoming 7th Grade EVW Student:

All children entering 7th grade must have proof of the following on file with the School Nurse prior the FIRST DAY of SCHOOL (September 5, 2023): for this grade a Tetanus booster after age 11,  and 1- Meningococcal (MenACWY),  also the previously required immunizations: 4 – Polio, 2 -MMR, the Hepatitis B series (3 shots), and either 2 Varicella (chickenpox) shots or the date they had the chicken pox disease.

Other immunizations recommended are: a COVID19 vaccine (1-2 shots), the Hepatitis A series (2 shots), and the HPV series (3 shots) vaccinations.

If your child has had the chickenpox, your health care provider needs to complete the section under the medical exemption “History of Varicella (chickenpox)”. Please also be sure to sign and have notorized the Student Immunization form below.

Children who cannot be vaccinated due to special medical problems, or whose parents are conscientiously opposed to immunizations, you may receive legal exemption. To receive legal exemption, parents need to file a medical exemption signed by their health care provider or a conscientious objection signed by them and notarized.

Sports Physical forms go to the Athletic Director, at the high school.

Over the summer You can send everything to the high school: 298 Brooks St N, Eden Valley, MN 55329  Attention: School Nurse.

 I will make a copy of the sports physical form for the student's health records & then will forward the original to the Athletic Director. For the Minnesota State High School League's Sports Physical Exam forms (see below) . Print and take pages 1-3 (possibly pg 4) with to your doctor's appointment. Page 2 should completed by a parent and student before the doctor's appointment. Page 1 is what we need returned to the school with a copy of their completed immunizations.
