Immunizations for 16 years and older / Entering 12th grade

Dear Parent/Guardian of a EVW Student 16 years and older:

It is required that all children 16 years and older receive their 2nd Meningococcal (MCV4 or MenACWY) immunization.

Other immunizations recommended if they haven't already received them are: the 1st Meningococcal B (MenB), COVID vaccine (1-2 shots), Hepatitis A series (2 shots), and the HPV series (3 shots) vaccinations.

Immunization information should be returned to the School Nurse. 

Please note that Sports Physicals go to the Athletic Director, and clinics don’t automatically update the school. Use this link for the MSHL 22-23 Sports Physical Form . Print and take pages 1-3 (possibly pg 4/5) with to your doctor's appointment. Page 2 should completed by a parent and student before the doctor's appointment. Page 1 is what we need returned to the school with a copy of their completed immunizations.
