Things to keep in mind:
IMMUNIZATIONS:
Parents should submit information about any immunizations received by their child to the School Nurse. Clinics do not automatically report to the school.
Students in Kindergarten should have the following immunization series complete: Hep B, Dtap/Tdap, Pneumococcal, Polio, MMR and Varicella complete.
Students in 7th grade will be required to have their Tdap updated.
Students in 12th grade will be required to have their Meningococcal immunization.
MEDICATIONS:
A new medication administration form must be completed at the beginning of each school year, for each medication, and each time there is a change in dosage or time of administration of a medication. *Prescription medication must be in a container labeled by the pharmacist or prescriber.
SPORTS PHYSICALS:
Students in grades 7th-12th that need a Sports Physical should bring the 2023-2024 Minnesota High School League Sports Physical Forms to their physician to complete and return the form to the Athletics Department (NOT the School Nurse), and a copy of immunizations to the School Nurse.

Use this form to determine if your child is up to date OR if you need to file an exemption, either medical or conscientious, with the school nurse.
Medication Form - Over the Counter (not prescribed by a doctor).
If you are sending any form of over the counter medications with your child to school, please complete this form. Medications need to have a label and your directions on how much and when to administer.
Medication Form - Prescription
If you are sending medications prescribed for your child to take during the school year, please have your doctor fill out and sign this form. You can also have your doctor provide a letter on their clinic letterhead. Both suffice for administration.
Special Diet Statement
If your child requires a special diet or food substitution, your physician must complete this form for our Health Office, the Dietary Department and your child's 504B.
MSHSL SPORTS PHYSICAL MEDICAL EXAMINATION
This form, once completed and signed by your child's physician, should be remitted to the Athletic Director (not the nurse's office).
