• Removing obstacles that prevent student learning

• Increase attendance, on time school-work completion

• Improve grades

• Connect every student/family to community resources to ensure successful completion of student plan

• Contact your Counselor if you would like to be referred for the program

Objectives:

• Connect daily to begin building relationships, connections to providers and community resources.

• Reduce family's barriers to accessing care and services

• Engage with community resources on behalf of family
