Use the Physical Education Unit Map to view the units our class will be participating in throughout the year.

Students please review the FMS Physical Education Syllabus with your parents.

Parents please complete the FMS Physical Education Syllabus FORM  acknowledging you have read and agree to the requirements of the syllabus.  

** ZERO PERIOD ONLY** Please complete, sign, print and return the Zero Period Contract .

Students please submit the PE ABSENCE ACTIVITY LOG  to earn your participation points if you have been absent, injured or sick.

In order to be excused from physical activity students must have a parent complete the  Physical Education Injury/Illness Form and provide the document to their PE teacher and the health clerk.
