REMINDER: The Associate Dean only process request for approved absence due to medical and mental health reasons.
Start processing your request for approved absences within 7 days, from the day you returned to your classes. The 7 days include the process with HSO and/or CPS.
Submit a request to the Associate Dean by filling up this form: Request for Approved Absences.
This form will require you to report the list of your courses, sections, faculty members, and date of absence, along with supporting documentation (i.e., HSO-validated medical certificate, email/certification from CPS)
After filling up the form the Associate Dean's Office will review your request and send an email to your professors within one working day after your submission.
If you have not received a response after one working day, please follow up with the office through email.
1. Present a hard copy of your medical certificate to the Health Services Office (HSO) to secure an endorsement within 7 working days from the day you returned to your classes.
2. Once you receive your medical certificate from the Health Services Office (HSO), create a digital copy and combine it with the original certificate into a single PDF file.
1. Schedule an appointment with the Counseling and Psychological Services (CPS) within seven working days from the day you returned to your classes.
2. CPS usually responds within three working days. If you do not receive a reply during that period, you may follow it up using the same email address.
3. Once you receive an email endorsement from CPS, print it as a PDF file. Submit this PDF file using the form "Request for Approved Absences"
1. Death of an immediate family member;
2. Emergencies;
3. Caregiving of sick family members;
4. Internet issues;
5. Sickness that does not usually require a medical certificate;
6. Delayed submission for course requirements;
7. Other special cases not mentioned above.
