NOTE:

• You need to sign in to your DepEd e-mail account to access the google form.

• In compliance with the Data Privacy Act of 2012 and its Implementing Rules and Regulations, we execute reasonable and appropriate security measures for the protection of personal data. 

• All information (personally identifiable or otherwise) collected and stored shall be used and processed by Northville 16 Elementary School for legitimate purpose.

• The Records Office does not accept requests made through phone calls or text.

• Request shall be verified based on existing records.

• Processing of documents will be minimum of 3 days and maximum of 10 days working process.

• You can claim your request via email & for hard copy of document/s you can visit the school.

• Records Office Hours: 8:00 am to 4:00 pm (Monday to Friday).
