Balarat

Fifth grade families,

Balarat Information: We have very exciting news! Our Balarat date has been announced! We will be attending Balarat November 4-5. The students have already heard, and should be coming home with a Balarat packet today.

Balarat is a 2-day excursion that is only offered to Denver Public Schools fifth graders. Balarat is only open to DPS staff so parents are not allowed on this excursion. We cannot take any chaperones for this excursion.

This trip requires a few things from you. Please read through the paperwork that came home with students today and return it as soon as possible.

White form: This is for you to keep. This has all of the information about Balarat and a packing list.

Yellow form: Please return the yellow form as soon as possible. This is the permission slip to attend Balarat. This form includes three different payment options for Balarat. The price is based off of your lunch eligibility status. We do not have access to your lunch eligibility status.

Pink/Purple form: This only needs to be filled out if you plan to send any medicine to Balarat. This includes medicine such as epi-pens, benadryl, Tylenol, etc. This form needs to be filled out by a doctor in order for medicine to be allowed at Balarat. Most doctors offices are very good about these kinds of forms, and will fill them out without a formal appointment. Additionally, Place Bridge Academy has a free clinic for all DPS students and can complete this form. All medicine will be held by the teacher. Students are not typically allowed to carry at Balarat. This form can be turned in after the yellow form if you need time to contact your doctor's office.

Please place all forms and money in the attached envelope and return to DGS as soon as possible.
