Each of the items included in the "Appendix" are items that are significant to the operation of our chapter or important to our individual members.  Current members should take some time to acquaint themselves with each document and/or form.  The forms  here are just examples.  If you need working copies of any forms you can obtain them from Room 123, Mrs. Johns' room.
