Family Income Information Form (FIIF) (Lunch Form)
As a reminder, the deadline for processing completed forms into the Online Data Acquisition (ODA) tool is Thursday, November 18. The FIIF (English/Spanish) is critical in determining the allocation of Supplemental Aid (SA) and Title I funds to your school for FY23. Additionally, it is frequently used as a needs-based data metric for eligibility or priority with other resources or funding. It is also used to screen for healthcare (Medicaid) and SNAP benefits for families.
School clerks should focus on the students that are NOT directly certified in ODA. Here are some helpful resources in case you need a refresher:

If you encounter a family that is NOT directly certified and has questions about their DHS Case number located on the FIIF, please refer them to the Healthy CPS Hotline, 773-553-KIDS (5437), where they will connected with a children and family benefits unit coordinator who will help them retrieve this information from the Illinois Department of Human
