Step 1. Seek any required urgent medical treatment immediately.
Your medical provider should be authorized by the New York State Workers Compensation Board (WCB). You can find an authorized provider at the link or phone number below:

Step 2. Call the CPMU President, VPO or VPA to report your incident as soon as possible.
Step 3. File a CPMU Incident Report
Step 4. File a claim with the New York State Workers Compensation Board (WCB):
Part A. Send WCB form VAW-1 and form C-3.1 to officers@cpmu.com (must be done within 90 days of incident)
Part B. Send WCB form VAW-3 to officers@cpmu.com and mail to the WCB (instructions are on the form)
Part C. CPMU will file your claim with our workers comp insurance provider, NYS Insurance Fund (volunteers do not file with NYSIF directly)
Part D. NYSIF will contact you directly after the claim is filed by CPMU.

Important Notes
DO NOT PAY ANY MEDICAL BILLS. Worker's comp claims must be paid directly by our insurance provider, NYSIF. NYSIF will contact you with information about how to submit bills.
All prescriptions must be filled using NYSIF's prescription benefits program through the CareComp Network of CVS Caremark. You can find the closest participating pharmacy by calling 866-493-1640 or ask NYSIF after the claim has been filed.
