The documents linked below for each certification program will provide potential candidates with information and details about the program.  None of these documents are needed for application, but a signed Candidate Agreement is required, with the first fee payment, once admitted to the program.

Application

Below are links to the CKEC Teacher Certification Institute application and reference forms.  You can either download the forms and return the completed forms by email or complete the forms online.  

Your 3 required references may do the same.   When selecting references, choose people who know you best, but as many as possible should be practicing or retired educators.

When completing the application please supply all information.  If accepted into the program, personal information you provide will be used for your account in the certification office at the Kentucky Department of Education/Education Professional Standards Board.

In addition to the forms, you must submit official copies of college transcripts.  The online form allows you to upload documents like transcripts.

At the end of the online submission process, you will be able to request a copy of your responses.

*Note: Those who currently hold a KY Teacher Certification are not eligible for this Program.
