DEADLINE #1: MEDICAL & MEDIA RELEASE FORM
What is it? A form providing emergency information including contact information, insurance information, and allergy information. It also includes an area to denote permission for the use of photo and video from this event to be used in the future.
Level of Importance: In order to bring a student Theatrefest, NTA must submit a Medical and Media Release Form and submit a list of names with every possible attendee and alternate. Festival administration and the university closely examine these forms for completion and send approval back to schools before the final field trip list is finalized and submitted. These forms do not guarantee or certify a spot on the trip, but without these forms turned in and certified with the Festival administration and university on time, a student will lose any eligibility to attend.
Special Notes: Every space on this form must be completed. All emergency contact and phone number spaces must be filled in with a response. In the case that a student does not have insurance, this must be marked on the form and cannot be left blank. Place N/A on any blank lines.

DEADLINE #2: WAIVER-BEARING STUDENT MEETING
What is it? A meeting for all prospective Fest participants (those who have turned in their waivers) to:
set the date & time for the MANDATORY parent meeting
launch our first fundraiser
discuss team interest in competing in the Tech Olympics
discuss interest in college/internship auditions and interviews
discuss interest in the International Thespy qualifier competition
review how reservation of spots on the trip works
answer any lingering questions
Level of Importance:
For most students, fundraising plays a critical role in being able to afford this 3-day (2-night) trip.
All Theatrefest attendees MUST have a parent representative attend the in-person parent meeting in order to take the trip. We need to determine how to make it as accessible as possible for your families.
Attending Theatrefest includes pre-registering for special events. We must submit these entries to the state Fest committee on a strict timeline!
If we have more interest than we have spots available, it is important to discuss how places on the trip are going to be reserved/determined, and how alternates are assigned.
Special Notes:
Fundraiser: Our Domino's fundraiser runs from November 14 through November 28th. The goal is for every student to sell 15 cards. Cards cost $20 and for each one sold, NTA Thespians receives $10. There is a sales form and an online store. The sales form is posted here for you to download.
If you will miss the meeting, you must reach out to us ahead of time and let us know. We will provide a summary email with all of the details that we discussed.

DEADLINE #3: First Deposit Due
What is it? Your first deposit is due!
Level of Importance: This is needed in order to make our trip reservations for each student.
Special Notes: Fundraising will offset your deposit amount.
January 06, 2025
DEADLINE #4: Final Balance Due
What is it? Your final trip balance is due.
Level of Importance: The finalized trip details go out to staff on this day, and it is essential to have all financial pieces in place before we can leave.
