Only Students Needing Committee Letters - Pre-Med/Dental/Podiatry - Need to Submit a Committee Interview Application

Go to Pre-Health Advisory Committee Interview Application to submit a formal request for a committee letter. You will need to submit the following:

• Brief Application Form

• 3 faculty letters of recommendation (ideally 2 from the sciences and 1 outside)

• Short response essays

• Your personal statement

• Unofficial Transcripts

• A typed 1-2 page resume (see how to write a resume for tips)

• A signed waiver form
