VFIS Beneficiary Form

Our firefighters are encouraged to keep their beneficiary information up to date in the event of accident or illness as a result of a call.  Members may fill out this form whenever a change is needed and submit it to the Board of Fire Commissioners via our secure mailbox or in-person at a monthly business meeting.
