Injury Paperwork

If a firefighter suffers an injury on a call, the injury must be reported within 24 hours. Notify the BOFC at the end of the call that an injury has occurred and followed the directions in the packet for reporting an injury to the township's worker's compensation company. Please print and complete these forms neatly and thoroughly with the supervisor that was on the call. Once finished, please submit the packet in the BOFC's secure mailbox and notify a commissioner that the forms are there so the Board can get them to the township promptly.
