The links below are for network managers when they are recruiting, hiring, and selecting a new staff member for their team. All forms will collect emails so only managers should use the forms below.
Posting and Sourcing for a Vacant Role: When looking to post or source for a vacant role, review all the core tenets to the BRICK Talent Selection Model.
School-based Stipend Request: Use this form to communicate with Talent regarding stipend agreements you would like presented to staff members. The agreement is sent through the HRIS for the staff member to sign. This request should be used for all centralized stipends that are used year over year.
For stipends not included in the above link, the manager should complete the stipend using the linked template. Keep in mind, this is a template that must be submitted to Talent for processing. Do not worry about logos or headings.
New Hire Request: Use this form to request an offer letter be generated for a new hire you and the hiring committee selected. There are three items required to be in the Applicant Tracking System (ATS) for the offer to be made:
At least 3 reference checks. These can be notes from a formal conversation and/or the reference check emailed response.
Verification of employment letters for any roles that are being counted as experience on the salary scale. Keep in mind, roles must be for the same or similar positions the new hire will be filling at BRICK.
Notes from each stage of the interview process. This should include phone interview notes, the artifact/project attached the ATS profile, and live interview notes.
New Employee Personal Information Request: This form must be completed by all new employees after an offer is accepted. Using the email template in the ATS (Lever) already has this form linked for a new hire to complete.
When adding a contractor operating like a full-time employee (FTE), follow the steps below:
Choose entity template (Where is this consultant being paid from?)
Fill in the Yellow highlighted areas of the contract
Fill out Schedule A on the contract template. (Sample)

If the contractor will be operating similar to a staff member (i.e., if they should be invited to staff events, receive announcements, get a company email, etc.), the contractor must complete their personal information and be added to the HRIS (Bamboo). Have the contractor complete this link.
New Position Request Form: This form must be completed when a manager is requesting a change or adjustment to a staffing model. This includes
Adding a brand new position (no job description on file)
Adding a new FTE to the Org Chart
Adjusting the role responsibilities & requirements for a current job description
Transition Plan: Use this form if an employee is transitioning out of a role and/or into a new role.
Onboarding & Orientation Plan: Use one of the templates, or a form you individually create, to think through onboarding & orientation of a new staff member. The important buckets to think through when onboarding are included in the templates shared below.
