As noted in the Employee Handbook, an incident report is a written report of any injury, accident, act of aggression, or unusual incident involving a student, staff, and/or individual.
Incident Reporting at BRICK
When an incident occurs, immediate notification to HR, regional leadership and network leadership is required. The form linked is to be used to automatically notify those individuals. immediate notification form linked here. This is a quick 2-minute submission and should be submitted immediately following an incident.
The more detailed BRICK Incident Form and Witness Statements should be forwarded to the school nurse and human resources department once collected (hr@brickeducation.org) . These are required within 24-working hours of the incident.
At the school level the Principal and DOO are responsible to ensure incident reports and witness statements are completed within the timelines noted above. At the network level it is the manager's responsibility.
Please note, if there is an injury, the worker's compensation section of the Handbook must be followed. Staff members must have a claim number from the Talent team prior to seeking approved medical care.

BRICK, SWPN, and BRICK Buffalo medical providers can be searched through this site. Log-in using the Username and password of TRINET. Select "find a provider" from the upper right.
An employee must provide the medical clearance prior to returning to work. All medical clearances should be emailed to hr@brickeducation.org

The process above detailed what is required as part of reporting. Once all documents are collected, and any investigations are completed, an incident must include follow-up
