Bloom Psychiatry Sliding Fee Discount Program & Good Faith Estimates

Whether or not you have insurance, Bloom Psychiatry may be able to reduce your costs through our Sliding Fee Discount Program. All patients are encouraged to complete an application. Eligibility is based on household size and income. Household size is the number of individuals inside or outside of the household, including the applicant, who is at least 50% dependent on the family income being reported on the application.

How much you will pay for services will depend on your income and household size. We offer discounts from regular cash rates based on income and household size.

New Patients: Please talk to our office manager, Laura, about the Sliding Fee Discount Program. She will help you determine your discount eligibility. 

To Apply for the Sliding Fee Discount Program please:

1. Speak with our office manager Laura, who will assist you in reviewing your insurance options.

2. Fill out the Bloom Psychiatry Sliding Fee Discount Program Application.

3. Provide proof of your estimated current annual income with:

   • Pay stubs, recent tax return, unemployment statements, SSI letter, child support statements, letter from employer, L&I award letter, bank statement, or a signed self-declaration of income

4. Proof of income is required annually to qualify for the Sliding Fee Discount Program. You are encouraged to complete a new sliding fee application if your household size changes or your monthly income changes by at least $150 and the change is expected to continue for at least two months.

Services Outside of Bloom Psychiatry

Bloom Psychiatry's Sliding Fee Discount Program only applies to services provided at Bloom Psychiatry. All other services provided by outside specialists, labs, or radiology are not covered and the patient will be responsible for any and all charges.

Bloom Psychiatry will not deny services based on a patient’s inability to pay, even if that means reducing or waiving costs (if applicable). Contact Bloom Psychiatry office manager at 509-557-0450 to learn about the resources available to you or to receive a Good Faith Estimate customized for you.

Important Notes: The Good Faith Estimate applies to uninsured patients who qualify for a sliding fee discount and is based on our understanding of your needs at application. While caring for you, our providers may recommend additional services that are not listed here. Your actual charges may vary from this estimate. This estimate is not a contract and does not require you to receive services from Bloom Psychiatry.
