What to Bring to Your Intake Visit
Arrive 15 minutes prior to your scheduled appointment. Check in with our office manager and provide your:
Identification: Valid photo ID (license or state ID)
Insurance Card
Completed Intake Forms: Demographic information, consent forms, release of information, other forms for initiating treatment.
Insurance & Payment
We accept many different insurances including commercial insurance, Medicaid (Washington State), and private pay. If insurance does not cover your care, we offer services on a sliding scale. Find out more about how you can afford the care you need.
