Technology Requirements
You will need the following tech items to complete a registration:
An internet connected device, preferably a computer or a tablet. Forms can be completed on a phone, but a larger screen is easier to read and a computer is easier for locating and uploading required documents.
An Internet browser, such as Chrome, Firefox, Safari.
We generally recommend using the Chrome browser. If you are using an Apple device (Mac, iPad, iPhone) and prefer to use the Safari browser, please refer to the directions at the bottom of this page to ensure your browser settings are correct.
If you have the PowerSchool app, you can use it to monitor your child's progress AFTER they are fully registered but not for registration.
A scanner or a smart phone with a camera to scan any paper documents. Directions are available on the next help page for scanning documents with a phone.
An email address you can send and receive emails from. The process cannot be completed without a functioning email address. Do not use a friend or relative's email address that you cannot access to retrieve or send mail.
Documentation Requirements
You must have all of the required documents BEFORE starting the online process. We recommend saving all of the required files in one folder on a computer drive, a flash drive, or a cloud drive, such as Google Drive or iCloud. Directions for scanning paper documents can be found in the next help section.
Download and print the Bloomfield Health Office Form and have it completed by your child's physician, including a full immunization report. This should be completed BEFORE you begin the online forms.
If you reside in Bloomfield but are not the lease or mortgage holder for the residence, download the Landlord Affidavit form, complete the form and have it notarized BEFORE you begin the online forms. If you are the lease or mortgage holder for the residence, you do not need to do this.
The child's birth certificate
Legal documents appointing custody or guardianship, if applicable
Previous school records, including a transcript for high school students; if applicable, IEPs and 504 plans will not be uploaded during registration but will be requested upon completion of the registration process
Either a mortgage statement or a current lease for the residence; if you are the owner, a tax bill for the property is also acceptable
If you are using the Landlord Affidavit form, the person who owns or leases the property must provide this document to you so you can upload it on their behalf.
THREE other proofs of residency that include your name and address, such as:
Electric bill
Gas bill
Phone bill (house or cell phone)
Television provider bill (cable, satellite, etc.)
Insurance bill
Voter registration card/records
Recent paycheck/stub
Driver's license with registration (both documents together count as ONE proof of residency)
Official mail (bank statement, government correspondence: Internal Revenue, Division of Taxation, Social Security Administration)
If you’re on an Apple device, we recommend using the Chrome browser, but if you prefer to use Safari, you’ll need to do the following:
For iPhone/iPad:
Open Settings
Scroll down to Safari
Scroll down to “Prevent Cross-Site Tracking” and make sure it’s turned OFF
For Safari on an Apple computer or laptop:
Open Safari
Go to the Safari menu and select Settings
Click on Privacy
Under “Website tracking”, make sure “Prevent cross-site tracking” is UNCHECKED
Please note: only use enrollment forms to initially register a student. If you have a change of address, phone number, or contacts please use the Information Update form on the General tab.
